NUTRITION
CONGREGATE
MEALS
This is a meal, prepared
in an approved kitchen, meeting USDA requirements, and served at a
nutrition site, senior center or some other congregate setting.
Eligibility: Any person
who is at least 60 years of age and the spouse of that person,
regardless of age.
Persons aged 18 59, with
disabilities IF:
1.
They reside in a housing facility occupied primarily by the
elderly at which congregate nutrition services are provided or
2.
They reside with a person 60 years of age or older AND accompany
that person to the congregate nutrition service site
Cost: None Voluntary
Contributions accepted
HOME DELIVERED MEALS
This is a meal provided to
an eligible person in their home. It is prepared in an approved kitchen
and meets USDA requirements.
Eligibility: Any
individual at least 60 years of age who is
homebound.
The spouse of that
individual IF it does not deny another homebound person a meal and
1.
The homebound person cannot be left alone or
2.
Receipt of the meal will enrich the physical, mental or emotional
well-being of the homebound person
3.
Persons aged 18 59 with disabilities IF:
a.
They reside in a housing facility occupied primarily by the
elderly at which congregate nutrition services are provided or
b.
They reside at home with a person at least 60 years of age
Cost: None Voluntary

MEDICAID WAIVER HOME DELIVERED MEALS
This is a meal
pre-authorized by the Division of Senior and Disability Services. It is
prepared in an approved kitchen and must meet 1/3 of the daily USDA
requirements.
Eligibility: The client
must be at least 63 years of age, on Medicaid and be pre-approved by the
Division of Senior and Disability Services
Cost: None
Contributions Cannot Be Accepted
